FOR MENTAL HEALTH COUNSELING SERVICES

We accept BC/BS and self-payments for all mental health counseling services offered in Virginia.

Blue Cross Blue Shield

Anthem BC/BS, Anthem Healthkeepers, CareFirst, BC/BS - Federal Employee Program, all other commercial (employer) BC/BS plans ‍ ‍

Crest Haven Counseling (Winchester) is in-network with BC/BS. Therefore, BC/BS will be billed (by CHC staff) for all services (as a courtesy service for our clients).

Crest Haven Counseling (Houston) is waiting for approval by BC/BS to bill for services.

Clients are responsible for co-pays and co-insurance (as determined by BC/BS) at the time of service.

Medicaid and Medicare

CHC is not approved by Medicaid or Medicare to bill for services. Therefore, all services for clients who are insured through Medicaid or Medicare will have to be paid for by the client (out-of-pocket).

Other Commercial (Employer or Market Place) Health Insurance Plans

CHC is out-of-network for all other insurance carriers. Therefore, all services for clients who are insured through another commercial (employer or Market Place) health insurance plan will have to be paid for by the client (out-of-pocket).

We are willing to provide superbills to our clients so that you may submit requests for reimbursement to your health insurance carrier.

Self-Pay Rates for Mental Health Counseling Services

$150 (for 45-55 minute counseling sessions)

$175 (for intake/initial assessment session only)

We also offer a sliding fee scale for qualifying clients.

and LIFE COACHING SERVICES

All Sport and Mental Performance Consulting and Life Coaching services must be paid for out-of-pocket.

We are not permitted to bill insurances for sport and mental performance consulting or life coaching services.

$150 (for 30-60 minute sessions)
